APPLICANT REQUESTS THAT: 

 

 1. Her Fitness Assessment (FA) dated 13 Dec 11, be removed 
from the Air Force Fitness Management System (AFFMS). 

 

 2. Her FA dated 12 Mar 12, be removed from the AFFMS. 

 

 3. The Letter of Reprimand (LOR) dated 12 Dec 11, be 
removed from her records. 

 

 4. The LOR dated 14 Mar 12, be removed from her records. 

 

 5. The Unfavorable Information File (UIF) (established due 
to items 1-4 above) be removed from her records.

APPLICANT CONTENDS THAT: 

 

During a recent medical appointment, her physician indicated that 
she should have been placed on a medical profile in Sep 2010 for 
medical issues. The profile should have been initiated when the 
medical problems first presented and she should have remained on 
a profile until further studies/treatment was rendered or a 
permanent waiver was obtained in order not to cause injury during 
the run.

STATEMENT OF FACTS: 

 

On 19 Dec 11, the applicant was issued a Letter of Reprimand 
(LOR) for receiving an Unsatisfactory FA on 14 Dec 11. On 22 Dec 
11, she acknowledged receipt of the LOR. 

 

On 24 Jan 12, she was notified by her commander that a UIF with a 
disposition date of two years had been established. 

 


On 14 Mar 12, she was issued a LOR for receiving an 
Unsatisfactory FA on 12 Mar 12. On 14 Mar 12, the applicant 
acknowledged receipt of the LOR.

AIR FORCE EVALUATION: 

 

HQ AFPC/DPSIM recommends approval of the applicant’s requests to 
remove the FA dated 13 Dec 11 and 12 Mar 12. DPSIM notes the 
applicant provided documentation which states she was not 
medically fit for her FA due to “snapping hip syndrome.” DPSIM 
also recommends the applicant’s LORs dated 19 Dec 11 and 14 Mar 
12 be removed from her records as both were issued as a result of 
the failed FAs.

Examiner’s Note: DPSIM states if the LORs were generated from 
the failed fitness reports; then they also recommend removal of 
the UIF.

APPLICANT'S REVIEW OF AIR FORCE EVALUATION: 

 

A copy of the Air Force evaluation was forwarded to the applicant 
on 15 Oct 12, for review and comment within 30 days. As of this 
date, this office has received no response (Exhibit C).

THE BOARD CONCLUDES THAT: 

 

1. The applicant has exhausted all remedies provided by existing 
law or regulations. 

 

2. The application was timely filed. 

 

3. Sufficient relevant evidence has been presented to 
demonstrate the existence of an error or injustice warranting 
relief. We note AFPC/DPSIM’s recommendation to remove the 
unsatisfactory FAs and LORs from the applicant’s record. In 
addition, we note DPSIM also recommends removal of the UIF. 
Therefore, we agree with DPSIM’s recommendation and recommend the 
applicant’s records be corrected as indicated below.

THE BOARD RECOMMENDS THAT: 

 

The pertinent military records of the Department of the Air Force 
relating to APPLICANT be corrected to show that 

 

 1. Her Fitness Assessments dated 13 December 2011 and 
14 March 2012, be, and hereby is, declared void and are removed 
from her records in the Air Force Fitness Management System. 

 

 2. Her Letter of Reprimand dated 19 December 2011 and 
14 March 2012, be, and hereby is, declared void and are removed 
from her records. 

 

 3. Her Unfavorable Information File (UIF) established on 
24 January 2012, be, and hereby is, declared void and removed 
from her records.
